Referring Ophthalmologist

1. Write letter with copies to general ophthalmologist, general practitioner etc.
2. If enucleation may be required, indicate in letter if this operation should be performed at referring hospital (otherwise surgery will be performed at Liverpool Ocular Oncology Centre).
3. Fax and post letter.
4. Phone oncology secretary, if referral is urgent.
5. Phone consultant oncologist if a discussion regarding the patient seems useful before referral.
6. Advise patient to get in touch with secretary if no appointment is received within ten days.

Patient
1. Contact referring ophthalmologist or general practitioner if appointment letter is not received within ten days.
2. Follow instructions in letter, for example, phoning oncology secretary if accommodation is required.
3. Complete and return questionnaire using stamped addressed envelope, to provide oncology secretary with information on general practitioner, optometrist, maiden name, religion and NHS number, if possible.
4. Contact general practitioner if ambulance is required (i.e., only if medically unfit to travel on public transport).

Oncology Secretary
1. Computerise information on receiving letter or phone call regarding referral.
2. Send patient (a) appointment letter from oncology secretary, (b) Information letter from oncology consultant, (c) information booklet regarding admission to hospital, and (d) personal details questionnaire.
3. Print patient information sheet from registration screen of database.
4. Give patient information sheet to clinic clerk for preparation of casesheet.
5. Write patient's name in diary.

Clinic Clerk
1. On receiving printed patient information sheet from secretary, register patient on SMS hospital system.
2. Prepare casesheet.
3. Add Initial Status Form and retinal diagram sheet to casesheet (3 pages).
4. Print patient information detail sheet and place in casesheet.
5. Store casesheet in clerk's room until morning of clinic.
6. A few days before clinic, print list of patients attending clinic.
7. On day of clinic, take casenotes to front desk.
